Under this support category, the NDIS aims to build self-confidence in people with disabilities to connect with a wide range of support services available within the community and health sector. Support and assistance are available to strengthen the abilities of participants in coordinating their existing NDIS supports, managing their development plan, making smooth transitions, and stepping into a new life stage with keenness.
We at Affection Disability Care acknowledge that transitioning to a new stage of life is a challenging time for anyone, and much harder for people with disabilities both emotionally and practically. Changes can be much harder, especially in new environments and settings. There are many transition points throughout a lifetime – starting school, undertaking educational courses, leaving home, getting that first job, moving home or into supported accommodation, and even getting out and about in the community more or retiring from work, for example. Not only that, we also support participants to plan for change, including developing strategies to cope with the stress that change can cause them. You can get the best outcomes out of each of these stages of life with a planned individualized approach that encircles individual abilities, choices, needs, and agreed outcomes. With proper planning and networking, we can support you through life’s transitions and on to achieving your goals both in the short term and long term. At Affection Disability Care, we excel at helping NDIS participants through life stage transitions.
Some of the support we provide includes the preparation of daily, weekly, and/or longer-term schedules. This is to help make sure that the support being provided adequately supports a participant’s goals of life stage transition and strengthens the participant’s ability to coordinate their supports, and to assist them to live comfortably and participate in their community. We can also help with the budgeting process so that expenses are planned for and paid for when due.
Under this NDIS support category, Affection Disability Care offers a wide range of supports to assist life stage transitions. The idea is to make this transition phase as smooth as possible without disrupting the comfort of the participant. We work with the participant to enable a friction-free move and encourage the participant to settle in a new setting and create a comfortable attitude. Some of our services for this purpose include:
