Support at Home (HCPs) are part of the Australian Government’s Aged Care system, designed to provide older Australians with long-term, coordinated support to help them live independently in their own homes for as long as possible.

How Support at Home Packages Work

  1. Assessment You begin with an assessment through My Aged Care to determine the level of support you require.
  2. Package Allocation Based on your needs, you will be assigned a package level (Level 1 to Level 8). These levels reflect the complexity and amount of support required.
  3. Choose Your Provider Once your package is available, you can select Affection Care as your trusted care provider.
  4. Personalised Care Planning We work closely with you and your family to create a personalised care plan that reflects your goals, preferences, and daily needs.
  5. Ongoing Support & Flexibility Your package funds essential services such as:
    • Personal care (showering, grooming, dressing)
    • Domestic assistance (cleaning, laundry, household tasks)
    • Meal preparation
    • Medication reminders
    • Social companionship
    • Transport to appointments
    • Allied health and wellbeing support
    • Safety and mobility equipment
  6. Consumer-Directed Care All Support at Home packages are delivered on a Consumer Directed Care (CDC) basis. This means:
    • You have choice and control over the types of care and services you receive
    • You can choose your provider, change providers, and manage how your funding is spent
    • You receive regular budget statements to stay informed about your package funds
